You need to activate Blood Frenzy before you can get additional stacks. Kills made while under the effects of Blood Frenzy generate additional stacks of Blood Frenzy.
I'm not sure if Blood Frenzy normally generates additional BF stacks when killing pets, but I'm not seeing where you turned the skill on before the slaying began.
