A swarm of bees (id 5714) while looking for spare ammo in dropped items in Urban Combat Simulation Center (65, 51), saw a drop-down listing "Sub-Machine Gun (worn) - 2". Picking it up once indeed gave a worn SMG. Looking at the drop-down again revealed an average SMG. Picking them both up and dropping them once again resulted in both the worn and average SMGs being combined into one listing, as if both items were of worn quality.

I can confirm this. Tossing around a bunch of throwing knives just leaves a bunch of Throwing Knife (average) on the ground. I can drop my destroyed Throwing Knives and then throw some average quality ones, and they are all mixed together with no option to just pick up the average quality. I need to pick them all up and then drop the destroyed ones in my inventory.
I get around this by dropping the broken and destroyed knives on a desert tile, but can confirm that this situation is happening.

There is similar stacking for spell gems for those without spellcraft. It is probably not a problematic behaviour so long as the best quality ones stay on the top of the stack (possibly repair grinders would want an option for worst quality on top).
It stacking is probably better for everyone's sanity (imagine the bookshop floor with 100 different guns listed) but it would be nice if the shown ones were based on quality rather than age/randomness.

Alas, the best quality do not necessarily stay on top. With my experiments with the average and worn SMGs, they inevitably combined into a stack listing as Worn, and picking one up gave me the worn one first.
